I see a problem in the word Jerusalem in 2 Kings 19:21 (last word). The final hireq shows up twice, once under the lamed and once under a circle as if it has no accompanyig consonant. It shows up that way in both Word 2007 and Internet Explorer 7. And also in this window: בַּ֖ת יְרוּשָׁלָֽ͏ִם. Is something wrong with the ordering here? The operating system is Windows Vista. Thanks for looking into this.

The problem here seems to be the presence of a combining grapheme joiner control character (U+034F) before the hiriq, which is breaking the expected contextual sequence for correct handling of the hiriq. If this control character is deleted, then the word renders correctly.

Is the presence of the CGJ character deliberate?
